Patient / Client Bill of Rights

As a client of TrustWise Care LLC, a Florida nurse registry, you have the following rights. These rights are derived from Florida Statutes 400.487 and 408.810 and apply throughout the time we serve you or your family member.

1. The right to be treated with dignity and respect

You have the right to be treated with consideration, respect, and full recognition of your individuality, including respect for your privacy in treatment and personal care.

2. The right to be informed

You have the right to be informed about your care, the credentials and training of any caregiver referred to you, and the costs you will incur. You have the right to receive a copy of these rights.

3. The right to participate in care planning

You have the right to participate in decisions about the care you receive, including the right to refuse any service, modify your care schedule, and request a different caregiver.

4. The right to confidentiality

You have the right to confidential treatment of all communications and records related to your care, subject only to disclosures required by law or made with your written consent.

5. The right to be free from abuse, neglect, and exploitation

You have the right to be free from mental, physical, sexual, and verbal abuse, neglect, and exploitation. If you believe you have experienced any of these, contact the Florida Abuse Hotline immediately at 1-800-96-ABUSE (1-800-962-2873).

6. The right to refuse service

You have the right to refuse any service from any caregiver, and to request a replacement caregiver at any time, without penalty.

7. The right to file a grievance or complaint

You have the right to voice grievances about your care or the conduct of a caregiver without fear of retaliation. You may speak with your TrustWise Care coordinator at any time. You also have the right at any time to file a complaint about any Florida nurse registry directly with the Florida Agency for Health Care Administration (AHCA) at 1-888-419-3456.

8. The right to be informed of complaint processes

You have the right to be informed of the procedures for filing complaints, both with us and with the Florida Agency for Health Care Administration.

9. The right to advance directives

You have the right to formulate advance directives and to have those directives respected within the scope of services we provide.

10. The right to be informed of independent-contractor status

You have the right to be informed that any caregiver referred to you by TrustWise Care LLC is an independent contractor, not an employee of the nurse registry, and that the caregiver contracts directly with you for services.

A copy of this Bill of Rights is provided to every client at the beginning of service. If at any time you have questions about your rights, please contact your care coordinator.
